• Har voter ko do Enumeration Forms diye jayenge.
• Door-to-door distribution 25 June se ek haftay tak hogi.

6. Tasveer Ki Zaroorat

7. Agar Naam Electoral Roll Mein Maujood Na Ho

• Agar kisi shakhs ka naam 2026 Electoral Roll mein nahin hai to woh 31 July ke baad Form-6 ke zariye enrollment ke liye darkhwast de sakta hai.

8. Agar Naam 2002 Roll Mein Na Mile

• Voter kisi aise qareebi rishtedaar ki maloomat de sakta hai jiska naam 2002 SIR Roll mein mojood ho.

9. Form Jama Karte Waqt

• BLO form receive karne ke baad ek copy par dastakhat karega.
• Voter acknowledged copy apne paas mehfooz rakhe.
• Yeh copy jama karwane ka saboot hogi.

10. Verification Aur Draft Roll

• Agar koi ghalti ya kami payi gayi to notice jari kiya ja sakta hai.
• Kuch voters online bhi form jama kar sakte hain.
• Draft Electoral Roll 31 July ko jari ki jayegi.
• Is ke baad claims aur objections jama karwaye ja sakte hain.

BLO Dobara Kyun Aayega?

• Enumeration Form dene ke liye.
• Agar ghar band ho to form pahunchane ke liye dobara koshish karne ke liye.
• Bhara hua form collect karne ke liye.
• Zaroorat par maloomat ki verification karne ke liye.

Voters Ke Liye Aham Hidayaat

• Form ko aaram se bharein; foran jama karna zaroori nahi.
• BLO jab form lene aaye to bhara hua form tayyar rakhein.
• Acknowledged copy zaroor hasil karein aur mehfooz rakhein.

Address Tabdeeli Karne Wale Voters

• Jo voters nayi jagah shift ho chuke hain woh apne maujooda pate par form bharein.
• Purane address wala EPIC Card address tabdeeli ki darkhwast ke liye istemal kiya ja sakta hai.

Agar Form Na Mile

• Agar voter ko form na mile to Electoral Registration Officer (ERO) ya Assistant Electoral Registration Officer (AERO) se rabta kare.

Aham Tanbeeh

• Form jama na karne par mazeed verification ho sakti hai.
• Jo voters absent, shifted, deceased ya duplicate report honge un ke naam draft roll mein shamil na bhi ho sakte hain.
• Aise afrad claims aur objections ke dauran apna case pesh kar sakte hain.

Asal Paigham

• Form durust aur mukammal taur par bharein.
• BLO ke saath taawun karein.
• Acknowledged copy sambhal kar rakhein.
• Draft Roll jari hone ke baad apna naam aur maloomat zaroor check karein.

SIR Rollout – Citizen Guide

1. Distribution of Forms

• Every voter will receive two Enumeration Forms.
• Door-to-door distribution will happen for about one week starting from June 25.
• Forms will be available in English in Hyderabad district.
• Forms will be available in Telugu in the rest of Telangana.
• If a house is locked, the Booth Level Officer (BLO) may visit up to three times to deliver the forms.

2. Filling the Form

• Fill the form carefully and accurately.
• You do not need to submit the form immediately.
• You will have about one month to complete it.
• The BLO will return later to collect the completed form.

3. Information Already Printed on the Form

The form will already contain:

• Name
• Address
• Photograph
• EPIC (Voter ID) Number
• Assembly Constituency Name
• Polling Booth Number
• Constituency and State of Enrollment
• QR Code

4. Information to Be Verified

Please check and confirm:

• Name
• Relative's Name
• Relationship
• District
• State
• Assembly Constituency

5. Additional Information Required

You may need to provide:

• Date of Birth
• Aadhaar Number
• Mobile Number
• Father's Name
• Mother's Name
• Spouse's Name
• EPIC Numbers of Family Members (if available)

6. Photograph Requirement

• Paste a recent photograph on the form.
• If you do not have a photograph, the BLO can take one.

7. If Your Name Is Not in the Electoral Roll

• If your name is not found in the 2026 Electoral Roll, you can apply for enrollment using Form 6 after July 31.

8. If Your Name Is Not Found in the 2002 Records

• You may provide details of a relative whose name appears in the 2002 SIR records.

9. Submission of Forms

• Submit the completed form to the BLO.
• The BLO will sign and acknowledge one copy.
• Keep the acknowledged copy safely as proof of submission.

10. Verification and Draft Roll

• Additional verification may be carried out if required.
• Some voters may also submit forms online.
• The Draft Electoral Roll will be published on July 31.
• Claims and objections can be submitted after the draft roll is published.

Why BLOs May Visit More Than Once

Delivery of Forms

• BLOs will visit homes to deliver Enumeration Forms.
• If a voter is unavailable or the house is locked, BLOs may visit again.

Collection of Completed Forms

• BLOs will make another visit to collect completed forms.
• Voters have about one month to fill and submit the forms.

Verification Visits

• Additional visits may be required to verify information.
• Further details or documents may be requested if needed.

For Voters Who Have Changed Address

• Voters who have shifted residence should fill the form using their current address.
• Existing EPIC cards can be used to request an address change.

If You Do Not Receive a Form

• Contact the Electoral Registration Officer (ERO) or Assistant Electoral Registration Officer (AERO).

Important Points

• Submit the form within the prescribed time.
• Keep the acknowledged copy safely.
• Check your details when the Draft Electoral Roll is published.
• Submit claims or objections if any information is incorrect.
